- I consent to FAST PSW collecting my personal information to create and manage my account.
- I consent to collection of personal health information needed to provide home care and PSW services.
- I consent to sharing necessary information with assigned PSWs and managers for my care.
- I consent to contact with family or emergency contacts when needed for safety or coordination.
- I consent to transportation support when I request that service.
- I consent to appointment coordination related to my care.
- I consent to app notifications, SMS, email, or phone calls about bookings and visits.
- I consent to service notes and care records being kept for quality and legal purposes.
I understand I may withdraw consent by contacting FAST PSW, subject to legal record-keeping requirements.
